Psychotherapy Assessment – Application Form

AGIP is offering assessments in person at the Fairbridge Clinic in Archway or on zoom. Many  of our members may also be willing to offer therapy via Zoom or FaceTime or telephone if you contact them directly. Please have a look at our members’ profiles and you are welcome to contact them directly.

The Fairbridge Clinic low fee scheme is only offering low fee psychotherapy to people who can commit to twice weekly therapy in-person for two years. However, the concessionary clinic offers more flexibility and video sessions are possible. This is considered on a case by case basis.

This form is for payment of an assessment with an AGIP assessor. The AGIP assessor can then recommend your next steps and, if suitable, refer you to a psychotherapist that will suit you.

There are two price levels for the consultation (£35 or £55). Please select the most suitable one (low fee or concessionary). You will be directed to PayPal to pay for the service when you click on Submit.

If you haven’t done so already, you will need to complete a short questionnaire before the assessment.
